2D is addicted to prescription pain medication. For good reason considering he's been hit by/thrown through the windshield of Murdoc's car more than once. I can't imagine his eyes feel very good. Painkillers are even listed as one of his "interests" in this Gorillaz MTV "Cribz" episode.
I think the scenario in this picture is that he got beat up by Murdoc and resolved to cry in the bathroom and TAKE SOME DRUGS. Kind of uh... makes me want to bang him. Because nothing says appreciation like surprise sex (AKA rape). Chicks dig it. EDIT: I fixed the fucking jagged pill bottle and bend in the thigh of the bent leg. See this image for differences.
